Forms for School

Each of these forms must be completed in order for your child to begin school.

Please complete one set of forms per child enrolled.

Please complete and print these forms and bring them with you to the

FORM DROP OFF Night.

Please follow each link below and complete the online forms prior to your child's MEET THE TEACHER Meeting.

Enrollment Form

ODJFS Required-this is different from the Registration form.

Child Medical Form

Must be updated every 12 months, signed by a doctor, and accompanied by current immunization records.

Food Allergy or Medical Condition Information

If your child has a food allergy or requires Montgomery Nursery School to monitor dietary restrictions or medical conditions, please call the office to schedule a meeting with the Director prior to school starting.
